Why the differential does the work

Identical numbers, and a total that hides the answer
Three of the five units on this page are the same number. A white cell count reported as 7.5 ×10⁹/L, 7.5 ×10³/µL and 7.5 K/µL is one identical result expressed three ways, because a litre is one million microlitres and the SI unit already carries a factor of 10⁹. Only the per-microlitre forms differ: 7.5 ×10⁹/L is 7,500 cells/µL, and the historical cells/mm³ is the same again, since a cubic millimetre is exactly one microlitre. A cell concentration is neither a mass nor an amount of substance, so there is no molecular weight and no molar unit at all.
The clinically important point is not the arithmetic but what the total conceals. The white cell count is the sum of five populations, and opposing shifts within it cancel out. A patient with a marked neutropenia and a compensating lymphocytosis can have a total sitting comfortably in the middle of the reference interval while being at real risk of overwhelming infection, and a normal total is therefore not reassurance on its own. The differential, and specifically the absolute counts derived from it, is where the information lives.
Severe neutropenia is defined on the absolute neutrophil count and not on the total. An ANC below 0.5 ×10⁹/L is the threshold at which fever becomes a medical emergency requiring broad-spectrum antibiotics within the hour, and a patient can reach it with a total white cell count that looks unremarkable — a total of 4.5 ×10⁹/L with 8 per cent neutrophils gives an ANC of 0.36 ×10⁹/L. Whenever leucopenia is reported, the absolute neutrophil and lymphocyte counts are the numbers to act on.
Two further cautions on the interval itself. It is strongly age-dependent: neonates run far higher and young children higher than adults, so applying the adult 4–11 ×10⁹/L range to a toddler over-calls the result. And people of African ancestry commonly have lower counts through a lower neutrophil fraction, a benign constitutional finding rather than a disease, which is repeatedly investigated as though it were one.

Is ×10⁹/L the same as ×10³/µL and K/µL?
Yes, all three are numerically identical — a count of 7.5 ×10⁹/L is 7.5 ×10³/µL and 7.5 K/µL. The only conversion on this page is to the per-microlitre forms: 7.5 ×10⁹/L is 7,500 cells/µL.
Is cells/mm³ the same as cells/µL?
Yes, exactly. A cubic millimetre is one microlitre by definition, so the older cells/mm³ notation and cells/µL are the same unit under two names and no conversion is needed between them.
Why is there no molar unit for a white cell count?
Because a cell concentration is neither a mass nor an amount of substance. There is no molecular weight to divide by, so mmol/L and µmol/L have no meaning here — the whole ladder is a matter of volume and powers of ten.
Does a normal white cell count mean everything is fine?
No. The total is the sum of five populations and opposing shifts cancel out, so a marked neutropenia with a compensating lymphocytosis can produce a completely normal total. Read the differential and the absolute counts, not the total alone.
Is severe neutropenia defined on the total white cell count?
