Reading the pattern

HBsAg — current infection
Anti-HBs — immunity
Anti-HBc — the person has met the virus itself, which is what separates vaccination from past infection

Isolated anti-HBc is the pattern most often misread. It has four possible explanations, and HBV DNA testing is the way to tell them apart.

Why this pattern is so often misread

Hepatitis B serology is one of the highest-volume searches in laboratory medicine, and the pattern is genuinely confusing on first encounter. The logic becomes tractable once each marker is tied to a distinct question. HBsAg answers whether the virus is present right now. Anti-HBs answers whether the person is immune. Anti-HBc answers whether the person has ever met the actual virus — and because vaccination contains no core antigen, anti-HBc is never positive from vaccination alone. That single fact is what separates immunity from vaccination (anti-HBs alone) from immunity following a resolved natural infection (both anti-HBs and anti-HBc positive).

Isolated anti-HBc — positive on its own, with HBsAg and anti-HBs both negative — is the pattern that causes the most difficulty, because it has four different explanations: the window period of a resolving acute infection before anti-HBs rises, a distant past infection whose anti-HBs has since waned below detection, occult chronic infection with HBsAg present below the assay’s detection limit, or a false positive result. HBV DNA testing is what distinguishes them, and it matters most before immunosuppressive treatment or rituximab, where reactivation of an occult infection can be fatal if unrecognised.

What does isolated anti-HBc mean?

Anti-HBc positive with both HBsAg and anti-HBs negative. It can mean the window period of resolving acute infection, distant past infection with waning anti-HBs, occult chronic infection, or a false positive — HBV DNA testing distinguishes them.

How do I tell vaccination from past infection on serology?

Anti-HBc. Vaccination never produces anti-HBc, because the vaccine contains no core antigen. Anti-HBs alone means vaccination; anti-HBs plus anti-HBc means a resolved natural infection.

What is the difference between acute and chronic hepatitis B on serology?

Both show HBsAg and anti-HBc positive. IgM anti-HBc positive indicates acute infection; IgM negative with a total anti-HBc positive indicates chronic infection.

Why does isolated anti-HBc matter before immunosuppression?

It can represent occult chronic infection with HBsAg below the assay’s detection limit. Immunosuppression, and rituximab in particular, can trigger reactivation of this occult infection, which can be fatal if not anticipated with HBV DNA testing and prophylaxis.
