The direction of the association is the single most important thing on this page, because it is the opposite of what a reader expects from a hormone made by fat. High adiponectin goes with leanness and insulin sensitivity; low adiponectin goes with obesity, insulin resistance and type 2 diabetes.

An abundant hormone with no decision limit

Adiponectin is secreted by adipocytes and is one of the most abundant hormones in plasma, circulating at micrograms per millilitre where most peptide hormones occupy picograms. That abundance is why the unit ladder here is a mass ladder with two pairs of identical units: micrograms per millilitre and milligrams per litre are the same number, nanograms per millilitre and micrograms per litre are the same number, and the two pairs differ by a factor of a thousand. The commonest error a reader makes with an adiponectin result is a thousandfold one, and it comes from not noticing which pair the report used.

There is no molar unit, and the reason is structural rather than conventional. The adiponectin monomer is a 30 kDa protein, but almost none of what circulates is monomeric. It assembles into trimers, then hexamers of relatively low molecular weight, then larger high-molecular-weight multimers, and the distribution between those forms is itself the biologically interesting variable. Work in both mice and humans showed that the ratio of high- to low-molecular-weight complexes, not the absolute amount of either, tracked improvement in insulin sensitivity during thiazolidinedione treatment, and that the high-molecular-weight complex is the active form. Dividing a mass concentration by the monomer’s molecular weight would therefore report the molarity of a species that is barely present.

The relationship with body fat runs in the direction most readers do not expect. Adiponectin is made by adipose tissue, and yet obese people have less of it than lean people — the finding was titled a “paradoxical decrease” when it was first described in 1999, and it has held since. Adiponectin is negatively associated with adiposity, with insulin resistance and with type 2 diabetes, and it rises with weight loss and with thiazolidinedione treatment. A low adiponectin is the abnormal direction here. Anyone reading an adiponectin result with the mental model they use for leptin, which rises with fat mass, will get the interpretation backwards.
