The rate of rise slows as hCG increases even in a normal pregnancy, which is why doubling time is only meaningfully interpretable below roughly 6,000–10,000 IU/L.

What doubling time can and cannot tell you

In a normal early intrauterine pregnancy, hCG roughly doubles every 48 to 72 hours up to about 6,000 to 10,000 IU/L, after which the rate of rise naturally slows as the trophoblast matures. Doubling time is therefore only a meaningful measure below that level; applying it to two high values from later in the first trimester will produce a misleadingly long number that reflects normal physiology, not a problem.

A rise of less than 35% in 48 hours is generally regarded as abnormal and raises concern for ectopic pregnancy or early pregnancy loss. But the overlap is real: around 15% of normal pregnancies rise more slowly than the classic rule, and around 15% of ectopic pregnancies rise at a rate that looks normal. hCG kinetics NEVER diagnose an ectopic pregnancy on their own — transvaginal ultrasound does, once hCG is high enough to make a pregnancy visible, and a slow or fast doubling time is a reason to arrange that scan rather than a diagnosis in itself.

A negative or very large result from this calculator means hCG fell or stayed roughly static between the two samples — that is, no doubling is occurring — rather than an error in the arithmetic. Also note that IU/L and mIU/mL are numerically identical, so no conversion is needed between laboratories reporting in either unit.

What is a normal hCG doubling time?

Roughly 48 to 72 hours in early pregnancy, up to an hCG of about 6,000 to 10,000 IU/L. Above that level the rate of rise slows naturally and doubling time stops being a useful measure.

Can hCG doubling time diagnose an ectopic pregnancy?

No, never on its own. About 15% of normal pregnancies rise more slowly than expected and about 15% of ectopic pregnancies rise normally. Transvaginal ultrasound, once hCG is high enough, is the diagnostic test.

What does a negative doubling time mean?

That hCG fell between the two samples rather than rose, so there is no doubling to calculate. It reflects the values entered, not a fault in the calculation.

Is mIU/mL different from IU/L for hCG?

No, the two units are numerically identical, so a result reported in either can be entered directly with no conversion.
