Why volume, not diameter, defines growth

The ellipsoid formula — volume equals 0.523 (π/6) multiplied by length, width and depth — is the standard method for estimating the volume of both discrete thyroid nodules and whole lobes on ultrasound, treating the structure as a rough ellipsoid.

The formula matters clinically because growth in the ATA and most surveillance guidelines is defined by volume, not by the largest diameter alone, and the two behave very differently. A 20% increase in each of the three dimensions produces roughly a 73% increase in volume, since volume scales with the cube of linear dimension. A nodule that looks only marginally larger on its longest-axis measurement may in fact have grown substantially once all three dimensions and their combined effect on volume are accounted for.

The American Thyroid Association defines significant interval growth as a 50% or greater increase in volume, or a 20% increase in at least two dimensions with a minimum increase of 2 mm — thresholds set deliberately above ordinary measurement variability, since inter-observer differences in ultrasound callipering are common and a small apparent change is often noise rather than true growth.

The formula assumes a regular ellipsoid, and it systematically overestimates the true volume of irregular, lobulated or multinodular glands, where the shape departs from that assumption. It is a screening and monitoring tool rather than a precise volumetric measurement, and should be interpreted alongside the sonographic pattern rather than as a number in isolation.
