Two units, one measurement

Two reporting systems for HbA1c exist side by side. NGSP/DCCT percentage is calibrated to the original Diabetes Control and Complications Trial and is the unit still used in the United States and in most historical literature. IFCC mmol/mol comes from a reference method developed after 2002 specifically to give HbA1c an SI-traceable, internationally reproducible number, and it is now the primary reporting unit in the UK, Australia, New Zealand and much of Europe. The two are related by a fixed linear equation, so nothing is lost by using either — a result in one unit converts exactly to the other.

The reason the numbers look so different is that they are not measuring quite the same thing. The IFCC reference method quantifies one specific, chemically defined species: the glycated N-terminal valine of the hemoglobin beta chain, measured by HPLC-mass spectrometry or capillary electrophoresis. NGSP-calibrated routine assays are standardised against this reference method but report a percentage of total glycated hemoglobin, a broader and less specific quantity historically. The master equation above is simply the empirical regression that ties the two calibration scales together.

In practice, a laboratory in a mmol/mol country and one in a percent country are reporting the same underlying glycaemic control; only the number on the page differs. The four thresholds above — 6.0%, 6.5%, 7.0% and 8.0% — are the ones that recur most often in guidelines, so having them fixed in both units avoids having to reconvert every time a report crosses borders.
