What the titre does and does not tell you
ANA is titrated by serial two-fold dilution and reported as the reciprocal of the last dilution at which fluorescence is still visible — 1:40, 1:80, 1:160, 1:320, 1:640 and so on. Because the series is a doubling one, a change of a single step sits inside normal assay variation and should not be read as a meaningful change in the patient; two steps or more is needed before a rise or fall means anything.
Low titres are common in health. Roughly 25 to 30% of healthy adults are positive at 1:40 and 10 to 15% at 1:80, so in someone without suggestive symptoms a low titre is far more likely to be a normal variant than early autoimmune disease. Ordering the test in a low-probability patient produces more false reassurance-seeking than useful information.
Titre also does not track disease activity in lupus and should not be repeated to monitor it — anti-double-stranded DNA antibody level and complement (C3, C4) do that job. What carries more diagnostic weight than the titre itself is the immunofluorescence pattern — homogeneous, speckled, nucleolar, centromere and others — since different patterns point toward different underlying antigens and different diseases.
Frequently asked questions
What does an ANA titre of 1:160 mean?
It means fluorescence was still visible after three doubling dilutions past the 1:40 screening dilution. On its own it is uncommon in health, but the pattern and the clinical picture matter more than the number.
Is a change from 1:160 to 1:320 significant?
That is only one doubling step, which is within normal assay variation. A meaningful change needs at least two steps.
Should ANA titre be repeated to monitor lupus activity?
No. ANA titre does not track disease activity. Anti-dsDNA antibody level and complement C3/C4 are the tests used to monitor lupus.
Is the titre or the pattern more important?
The pattern. Homogeneous, speckled, nucleolar and centromere patterns point toward different antigens and different diseases, and carry more diagnostic information than the titre alone.
