In clinical pathology, elevated acetone levels are a hallmark of ketoacidosis (diabetic, alcoholic, or starvation). Unlike other ketone bodies (beta-hydroxybutyrate and acetoacetate), acetone is volatile and is excreted via the lungs, giving the breath a characteristic “fruity” odor. While it does not contribute significantly to the acid load, its presence confirms a state of ketosis. Acetone is the least abundant of the three ketone bodies in the blood (after beta-hydroxybutyrate and acetoacetate). It is often measured in toxicology screens rather than standard metabolic panels to distinguish between different causes of metabolic acidosis.
In Diabetic Ketoacidosis (DKA), serum acetone levels can rise significantly. Importantly, acetone is a primary metabolite of Isopropanol (rubbing alcohol). Finding high acetone levels in the absence of severe acidosis (and absence of elevated beta-hydroxybutyrate) is a strong indicator of isopropanol ingestion. Normal serum levels are typically < 1 mg/dL.
